29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. (a) Roof - On or about April 1, 2014 an employee was exposed to fall hazards while working on a metal building roof near a skylight. Employee was not protected from falling 21.5 feet through the skylight to concrete floor by personal fall arrest systems, covers or guardrail systems.

29 CFR 1904.39(a): The employer did not orally report the death of an employee or the in-patient hospitalization of three or more employees because of a work-related incident: (a) Jobsite - On or about April 1, 2014 an employee work related death occurred at 12:15 pm. An oral report of the death was not reported to the nearest OSHA office within 8 hours.
