29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection.     Located at the middle condominium unit workers utilized a Porter Cable circular saw that was connected with a 50.0 foot yellow extension cord to the electrical service to cut wooden framing material. Workers were exposed to potential electrocution hazards in that the electrical service was not protected with a ground-fault circuit interrupter nor was an assured equipment grounding conductor program in use.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.          Workers were engaged in commercial construction activities installing wooden framing material on new multi-story condominiums. Workers utilized a JLG 600S articulating, extensible, boom aerial lift and were exposed to 40.0 foot fall hazards in that the workers were not attached to the basket or mast.
