29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): (a) On or about April 3, 2014 at worksite 14201 Botts Road, Grandview, Missouri: employees in the injection mold area were exposed to a serious struck by hazard from the Toyota Forklift that was in operation, without the rubber cover on the brake pedal. Abatement certification is required for this item.
