29 CFR 1926.502(f)(3): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a) Jobsite, Roof: On or about April 1, 2014, three employees were working near the edge of the roof and they were not protected from falling off of the roof. The fall hazard to the employees was 25 feet to the ground.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) Jobsite: On or about April 1, 2014, the employer did not provide a training program for employees who are exposed to fall hazards. There were three employees on this site that were exposed to fall hazards of approximately 25 feet from the roof.
