29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening was less than 39 inches (1.0 m) above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system: a) Job site, 4750 Clairton Boulevard, Pittsburgh, PA: On or about June 26, 2018, during construction activities, the employer did not ensure that appropriate fall protection/systems were in use where an employee was working in front of an open window that allowed for a wall opening of approximately 33 inches wide and 48 inches high to exist above an approximate 32 1/2 inch tall window sill, exposing the employee to a fall hazard of up to at least approximately 12 feet.
