29 CFR 1910.95(c)(1): A continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (n) was not instituted when employee noise exposures equaled or exceeded an 8 hour time weighted average sound level (TWA) of 85 dBA:  For the Plasma Cutter at the Plasma Table who was exposed to a recorded personal noise dosimeter level of 88.2% on 4/3/2014.  This was equivalent to a sound level of 89.1 dBA measured over an 8-hour time-weighted average (TWA).        The Construction Trailer Specialists, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.95(c)(1), which was contained in OSHA inspection number 314737578 citation number 2, item number 1 and was affirmed as a final order on November 18, 2010, with respect to a workplace located at 2535 Rose Parkway, Sikeston, MO  63801-1193.
