29 CFR 1910.147(c)(6)(i)(A): The periodic inspection of the energy control procedure was not performed by an authorized employee other than the one utilizing the energy control procedure being inspected.   On or about 2/3/2023, Construction Trailer Specialist, Inc. did not conduct a periodic inspection of the energy control procedures at least annually to ensure that the procedures and the requirements of this standard were being followed. The last verified energy control procedures review was conducted in June 2019.

On or about 2/3/2023, Construction Trailer Specialists, Inc. had not ensured the personal fall protection system was inspected prior to use during each shift for wear, damage, or defective components and were not removed from service.  The securing pins to the vertical structural members of the fall protection anchorage system were not secured by cotter pins and not been removed from service until the defective condition was repaired.  Construction Trailer Specialists, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 19210.140(c)(18), which was contained in OSHA inspection number 1333092, citation number 1, item number 2 and was affirmed as a final order on 8/31/2018, with respect to a workplace located 2535 Rose Parkway in Sikeston, MO

The manufacturer's identification and lifting capacity was not legible. b) Nissan 50 (#7) SN-E6003. The manufacturer's identification and lifting capacity was not legible. c) Nissan 50(#9) SN E200621.  The manufacturer's identification and lifting capacity was not legible. On or about 2/3/2023, Construction Trailer Specialist, Inc. had not ensured any powered industrial truck which was found to be in need of repair, defective, or in any way unsafe, was removed from service.  The following forklifts were in use and not removed from service:  a) Nissan 50 (#4) Model MP1F2A25LS, SN P1F2-9H2008.  The forklift was defective at the time of the inspection; it was missing the accelerator pad on the pedal.  The pedal had bare metal exposed. The PIT makings are not legible. b) Nissan 50(#9) SN E200621.  The forklift was defective at the time of the inspection; it was missing the accelerator and brake pads on the pedals.  The PIT Identification labels are not legible. c) Nissan 50 (#7) SN-E6003. The forklift was defective at the time of the inspection; it was missing the accelerator pad on the pedal.  The pedal had bare metal exposed.  The PIT Identification labels are not legible. d) Nissan (#6) Model CPJ02-98618.   The forklift was defective at the time of the inspection; it was missing the accelerator pad on the pedal.  The pedal had bare metal exposed.
