29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven feet or less above floors or platforms were not enclosed: On or about April 2, 2014, the employer did not ensure that rotating chain sprockets were properly guarded. Carts #58, 50, and 41 were missing the guard at the rotating chain sprocket. This condition exposed employees to caught-in hazards.

29 CFR 1910.303(b)(7)(iv): There are damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating. On or about April 2, 2014, employer did not ensure that electric cart #50 acceleration pedal cover was installed before being operated. This condition exposed employees to electrical hazards.
