29 CFR 1910.333(b)(2)(iii)(A): A lock and a tag were not placed on each disconnecting means used to de-energize circuits and equipment on which work was to be performed: (a) A worker who had not been provided locks in order to de-energize and lock out conductors to the overhead crane, sustained an electrocution injury working in close proximity to live circuits.
