OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:  On or about October 17, 2024, employees performing outdoor material handling using the Hyster Forklift 2651, on a gravel roadway, at 802 Allied Road, Bellevue, Nebraska were exposed to caught-between and crushing hazards in the event of a tip-over and ejection from the operator's station.

29 CFR 1910.184(c)(9): Employee(s) were not kept clear of load(s) suspended, or about to be lifted by sling(s):  On or about October 7, 2024, it was documented that employees engaged in pretensioning activities at Coreslab Structures (Omaha) Inc. located at 802 Alliance Road, Bellevue, Nebraska were exposed to struck-by hazards in that employees manually braced and/or manipulated hydraulic jacking rams during operation while they were suspended from an overhead crane.  Instances included, but were not limited to:  i. Daily operations in building 1 where employees perform initial pretensioning to approximately 3,000 pounds per square inch and ii. Daily operations in building 2 where employees perform initial pretensioning to approximately 3,000 pounds per square inch.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   On or about October 2, 2024, it was documented that employees engaged in pretensioning activities at Coreslab Structures (Omaha) Inc. located at 802 Alliance Road, Bellevue, Nebraska were exposed to caught-in hazards in that the employer did not ensure the hydraulic jacking ram was guarded to protect employees and prevent them from placing body parts into the hydraulic ram during operation.  Instances included, but were not limited to:  i. Daily operations in building 1 where employees perform initial pretensioning to approximately 3,000 pounds per square inch and ii. Daily operations in building 2 where employees perform Initial pretensioning to approximately 3,000 pounds per square inch.

Instances included, but were not limited to, the synthetic web slings used during:  i. Daily operations in building 1 where employees used the hydraulic jacking ram; G. T. Bynum CO. Model M P U28, Serial number: 122297 and ii. Daily operations in building 2 where employees used the hydraulic jacking ram; G. T. Bynum CO., Model: M P U2, Serial number: 111396.

Instances included, but were not limited to the synthetic web slings used during:  i. Daily operations in building 1 where employees used the hydraulic jacking ram; G. T. Bynum CO. Model M P U28, Serial number: 122297 and ii. Daily operations in building 2 where employees used the hydraulic jacking ram; G. T. Bynum CO., Model: M P U2, Serial number: 111396.

Instances included, but were not limited to the synthetic web slings used with:  i. Daily operations in building 1 where employees used the hydraulic jacking ram; G. T. Bynum CO. Model M P U28, Serial number: 122297 and ii. Daily operations in building 1 where employees used the hydraulic jacking ram; G. T. Bynum CO., Model: M P U2, Serial number: 111396.
