29 CFR 1910.22(d)(1): On every building or other structure, or part thereof, used for mercantile, business, industrial or storage purposes, the loads approved by the building official were not marked on plates of approved design and securely affixed in a conspicuous place in each space to which they relate.  (a) Front Entrance:  Employees were exposed to overhead hazards, such as, but not limited to, the overhead storage on the second floor above the balers and areas where employees work, was not marked with a load rating.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.  (a)  Second story, Front entrance:  Employees were exposed to potential falls from heights as the second story did not have standard railings.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) Baler Area:  Employees were exposed to the Green Baler, which had General Hydraulics listed on the control box. The safety switch had been wired back creating point of operation hazards as it bypassed the feature to keep the gate closed while the baler was in operation.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  (a) Throughout facility:  Employees were exposed to hazardous chemicals, such as, but not limited to, sulfuric acid contained in batteries, which employees were exposed to during the recycling process.  There was no written hazard communication program describing how hazardous chemicals were to be labeled, how safety data sheets were to be obtained, and how employees were going to be provided with training on hazardous chemicals.
