29 CFR 1910.106(d)(4)(i): Inside storage rooms for flammable and combustible liquids had openings to other rooms or buildings that were not provided with noncombustible liquid-tight raised sills or ramps at least 4 inches in height; or the floor in the storage area was not at least 4 inches below the surrounding floor; or the inside of the room did not have an open-grated trench which drained to a safe location.  Openings from inside flammable and combustible liquid storage rooms were not provided with approved self-closing fire doors:  (a) Assembly Services, Flammable and Combustible Liquid Storage Rooms behind the Paint Booths - The employer had two inside storage rooms for storing flammable and combustible liquids, including, but not limited to, PPG Shop-Line Slow Reducer JR507, PPG Shop-Line General Purpose Solvent JT501, and PPG Shop-Line Wax and Grease Remover JX101 (all Category 2 flammable liquids).  These inside storage rooms did not have either noncombustible liquid-tight raised sills or ramps at least 4 inches in height; or floors in the storage rooms at least 4 inches below the surrounding floor; or the inside of the room did not have an open-grated trench which drained to a safe location.  In addition, the opening to each of these inside storage rooms had double doors, but only the right side door of each set of doors was self-closing, exposing employees to a fire hazard, on or about April 8, 2014.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.106(e)(6)(ii): Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 degrees F (37.8 degrees C), were dispensed into containers without the nozzle and container being electrically interconnected:  (a) Assembly Services, Flammable and Combustible Liquid Storage Rooms behind the Paint Booths - Employees dispensed PPG Shop-Line Slow Reducer JR507 (a Category 2 flammable liquid) from a 55-gallon drum to a 5-gallon container.  The container had a plastic opening which prevented the bonding of this container to the nozzle on the 55-gallon drum, and employees did not use a bond wire to electrically bond the 55-gallon drum to the 5-gallon container, exposing employees to a fire hazard, on or about April 8, 2014.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.106(e)(9)(iii): Combustible waste material and residues in a building or unit operating area were not kept to a minimum, stored in covered metal receptacles and disposed of daily:  (a) Assembly Services, Paint Booths - Inside the paint booths, the employer had ordinary plastic garbage cans filled with combustible waste, including, but not limited to, a used up can of PPG Shop-Line General Purpose Solvent JT501 (a Category 2 flammable liquid), used sanding discs, and used Tyvek coveralls.  In addition, the employer kept a metal 55-gallon drum in each paint booth for employees to use to dispose of waste PPG Shop-Line General Purpose Solvent JT501 (a Category 2 flammable liquid).  Neither the garbage cans nor the waste solvent drums got emptied on a daily basis, exposing employees to a fire hazard, on or about April 8, 2014.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  (a) Keen Transport, Inc., 1951 Harrisburg Pike, Carlisle, Pennsylvania - Employees did servicing and maintenance work on machinery and equipment, including, but not limited to, repairing and maintaining trucks and trailers and setting up equipment by adding pre-lube kits, hydraulic kits, etc., and the employer did not conduct an annual inspection of the energy control procedures to identify and correct any inadequacies or deviations, exposing employees to caught-in, struck-by and other hazards resulting from unexpected energization, on or about April 8, 2014.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
