29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as follows: (a) Back of Building - On or about April 3, 2014 an employee was exposed to a fall hazard due to a scaffold not being fully decked or planked. One deck was not installed on the scaffold resulting in a 21.5 inch gap where the employee could fall 15 feet on to concrete.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. (a) Back of Building - On or about April 3, 2014 an employee was exposed to a fall hazard while standing on a scaffold 28 feet above concrete with no guardrail system or personal fall arrest system. (b) Back of Building - on or about April 3, 2014 an employee was exposed to a fall hazard while standing on a scaffold 15 feet above concrete with no guardrail system or personal fall arrest system.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used. (a) Back of Building - On or about April 3, 2014 two employees were exposed to a fall hazard due to a scaffold not having a ladder installed. This scaffold was measured at 28 feet above concrete. Stucco Brothers, LLC was previously cited for a violation of this occupational safety standard 29 CFR 1926.451(e)(1), which was contained in OSHA inspection number 896597, citation number 1, item number 2 and was affirmed as a final order on May 21, 2013, with respect to a workplace located at 526 Evergreen Street, Ridgeland, MS 39157.

29 CFR 1926.451(f)(7): Scaffolds was not erected, moved, dismantled, or altered under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration. (a) Back of Building - On or about April 3, 2014 two employees were exposed to a fall hazard due to the fact the scaffold was not erected and altered under the supervision of a competent person. This scaffold was missing vital components such as: guardrails, metal decks and a ladder.
