Site - On or about March 27, 2014, an employee assigned to operate the Delta Rockwell (model # 34-466 S/N 1Y9310) table saw, suffered a finger amputation while operating the saw without an automatically adjusting hood blade guard in place. Instance b. Site - On or about April 7, 2014, an automatically adjusting hood blade guard was not provided for the Powermatic (model 66 S/N 9663141) table saw in use.
