29 CFR 1910.213(h)(1): The sides of the lower exposed portion of the blade of radial saw(s) were not guarded to the full diameter of the blade by a device that automatically adjusted itself to the thickness of the stock and remained in contact with the stock being cut to give maximum protection possible for the operation being performed: Instance a. Site - On or about April 28, 2015, an employee assigned to operate the Omca (model RN450FM "US") radial arm saw suffered a serious finger injury, while operating the saw which had not been adequately guarded. Instance b. Site - On or about April 28, 2015, the Omca (model RN450FM "US" S/N 280662) radial arm saw's blade guard was not installed correctly to allow the guard to completely enclose the rotating blade during the operation of the saw. Instance c. Site - On or about April 28, 2015, the Delta Rockwell (S/N 2556-B) radial arm saw's blade guard was not installed correctly to allow the guard to completely enclose the rotating blade during the operation of the saw. Vigilant Incorporated was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.213(c)(1), which was contained in OSHA inspection number 966547, citation number 1, item number 2, and was affirmed as a final order on 5/23/2014, with respect to a workplace located at 85 Industrial Park Dr. Dover, NH.

Site - On or about April 28, 2015, and May 7, 2015, the Hitachi (model C10FSH S/N 05045A) slide compound saw's vise assembly was not utilized to secure the workpiece and prevent cut materials from being thrown backwards towards the operator. Instance b. Site - On or about April 28, 2015, and May 7, 2015, the Hitachi (model C10FSH S/N CD40024) slide compound saw's vise assembly was not utilized to secure the workpiece and prevent cut materials from being thrown backwards towards the operator. Instance c. Site - On or about April 28, 2015, and May 7, 2015, the Hitachi (model C10FSH S/N 350565) slide compound saw's vise assembly was not utilized to secure the workpiece and prevent cut materials from being thrown backwards towards the operator. Instance d. Site - On or about April 28, 2015, and May 7, 2015, the Hitachi (model C10FSH S/N 571317) slide compound saw's vise assembly was not utilized to secure the workpiece and prevent cut materials from being thrown backwards towards the operator. Instance e. Site - On or about April 28, 2015, and May 7, 2015, the Hitachi (model C10FSH S/N 640995) slide compound saw's vise assembly was not utilized to secure the workpiece and prevent cut materials from being thrown backwards towards the operator. Instance f. Site - On or about April 28, 2015, and May 7, 2015, the Hitachi (model C10FSH S/N 970901) slide compound saw's vise assembly was not utilized to secure the workpiece and prevent cut materials from being thrown backwards towards the operator. Instance g.
