Instance c. Site - On or about April 28, 2015, the Delta Rockwell (S/N 2556-B) radial arm saw's blade guard was not installed correctly to allow the guard to completely enclose the rotating blade during the operation of the saw.        Vigilant Incorporated was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.213(c)(1), which was contained in OSHA inspection number 966547, citation number 1, item number 2, and was affirmed as a final order on 5/23/2014, with respect to a workplace located at 85 Industrial Park Dr. Dover, NH.
