29 CFR 1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces  At Shoupe Plant, at the F-3 vessel separator unit, the host employer did not inform or warn contractor employees who entered the vessel of the existence and potential hazards by means of posting danger signs or other equally effective means. Contractor employees were exposed to potential atmospheric hazards created by the existence of residual product and other chemicals such as, but not limited to, iron oxide.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met  At Shoupe Plant, the employer did not implement its hazard communication program to contractor employees who were working at the f-3 filter separator unit. The employer did not provide effective information and training on hazardous chemicals in their work area such as, but not limited to, iron oxide and product residue in and around the vessel. Contractor employees were not informed aware of the chemicals present at their work site.
