On or about 8/02/2016, at the Enterprise Products Operating facility located in Pasadena Texas, the employer did not document that equipment complied with recognized and generally accepted good engineering practice such as American Society of  Mechanical Engineers (ASME) Boiler and Pressure Vessel Code Section VIII, Division 1 (2007  employer documented RAGAGEP for pressure vessels), UG-125(d), UG-135(d), and Appendix M-5, when pressure vessels had the potential to be operating with an obstructed relief path due to the employers failure to implement administrative controls and locking devices on intervening block valves for the following pressure relief devices and their associated pressure vessels:       a.  Block valve for PSV-1121 which protects pressure vessel R110 as depicted on P&ID F-11-00112 Revision #12    b.  Block valve for PSV-1122 which protects pressure vessel R111 as depicted on P&ID F-11-00113 Revision #8     c.  Block valve for PSV-1131 which protects pressure vessel R112 as depicted on P&ID F-11-00114 Revision #7      This condition exposes workers to the hazards of a catastrophic equipment failure including being struck by a high energy release, equipment parts, and fire/explosion.
