29 CFR 1910.119(e)(5): The employer did not assure that PHA recommendations were resolved in a timely manner  (a)  Cryogenic Plant, Train A - On or about August 22, 2016 Enterprise Product Partners L.P., did not have available detailed documentation of resolutions for items listed in the PHA Recommendation Action Report dated September 7, 2009.  Employees were exposed to fire and explosion hazards at the aluminum exchangers which had been recommended by the PHA team to evaluate replacing H-3052/3159, H-3059/3159 exchangers with tube and sheet exchangers.

29 CFR 1910.119(l)(1): The employer did not establish and implement written procedures to Management of Change (except for "replacements in kind") to process chemicals, technology, equipment, and procedures; and, changes to facilities that affect a covered process.    (a) Cryogenic Plant, Train A - On or about August 27, 2016 the employer did not have documentation showing that consideration, such as the technical basis for the change; it impact on employee safety and health; modification of operating procedures; time period of change and authorization for the proposed change had been address prior to changing a 4 low temp carbon steel upstream gate valve (LV 1905) for a stainless steel valve identified by eMOC 769.            (b) Cryogenic Plant, Train A - On or about August 27, 2016 the employer did not have documentation showing that consideration, such as the technical basis for the change; its impact on employee safety and health; modification of operating procedures; time period of change and authorization for the proposed change had been address prior to pressure valve 58-PV5702A identified by eMOC 7750.
