29 CFR 1910.176(a): Where mechanical handling equipment was used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made, aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard and permanent aisles and passageways were not appropriately marked: Instances including, but not limited to the following: a) Jobsite, 6th and Center Street, Tarentum, PA: On or about April 30, 2014, during production activities, the employer did not ensure that forklift travel paths around K-plant and the warehouse areas were kept in good repair in that potholes and ripples in the ground existed.

29 CFR 1910.22(d)(1): In every building or other structure, or part thereof, used for mercantile, business, industrial, or storage purposes, the loads approved by the building official were not marked on plates of approved design which will be supplied and securely affixed by the owner of the building, or his duly authorized agent, in a conspicuous place in each space to which they relate, and these plates will not be removed or defaced but, if lost, removed, or defaced, they were not replaced by the owner or his agent: a) Jobsite, 6th and Center Street, Tarentum, PA: On or about April 30, 2014, in the section of the warehouse that faces the administrative facility and/or K Plant, the employer did not ensure that floor load limit signage was in place. Part of the floor in this area had collapsed, causing stability concerns, and the floor load rating was not known, yet operation of forklifts and the storage of heavy materials was still practiced in this area.
