29 CFR 1926.651(e): Employee was not prohibited to be underneath loads handled by lifting or digging equipment:  At this excavation site, an employee was installing new piping to a drain line.  An employee stood underneath the claw of an excavator as it conducted excavation operations.  The employee was exposed to struck-by hazards.

29 CFR 1926.651(j)(2): Employees were not protected from excavated or other materials or equipment that could pose a hazard by falling or rolling into excavations, or by a combination of both if necessary:  At this excavation site, employees were installing a drain line to an existing street and the spoil pile was within 1 feet from the edge of the trench.

29 CFR 1926.652(b)(2): Option (2) - Determination of slopes and configurations using Appendices A and B. Maximum allowable slopes, and allowable configurations for sloping and benching systems, shall be determined in accordance with the conditions and requirements set forth in appendices A and B to this subpart:  At this job site on Bass Pro Drive, an employee worked in a trench approximately 20 ft. wide x 8 ft. deep x 40 ft. long.  The dirt walls of the trench were not benched in accordance with Appendix B above.
