29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) The rotating nut ends on the Buffalo Forge Co., Bending Roll were not guarded to prevent employees from accessing the danger area while the machine is in operation. This created an entanglement hazard.

29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: (a) The point of operation on the Buffalo Forge Co., Bending Roll was not guarded to prevent employees from reaching the danger zone while the machine was in operation. This exposed employees to amputation hazards.

29 CFR 1910.138(b): The employer did not base selection of appropriate hand protection on an evaluation of the performance characteristics of the hand protection relative to the tasks to be performed, conditions present, duration of use, and the hazards and potential hazards identified: (a) The employer failed to ensure the cut resistant gloves selected for employees who operate the Buffalo Forge Co., Bending Roll was adequate to prevent entanglement when employees were feeding parts.
