29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used: On or about April 8, 2014; at the center of the jobsite, the employer allowed employees to utilize a damaged extension cord in order to power a hand-held spade handle mixer. Employees used the spade handle mixer to mix and prepare mortar for application.

29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: On or about April 8, 2014; at the center of the jobsite, the employer allowed employees to utilize extension cords in a manner that was not in accordance with instructions included in the labeling. Employees connected electrical extension cords together to create a chain of three (3) extension cords in order to power a hand-held spade handle mixer. Employees used the spade handle mixer to mix and prepare mortar for application.
