29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form or scrap lumber with protruding nails was not kept clear from work areas: a) On or about April 9, 2014 at 365 Broadway, Saratoga NY 12866 a piece of debris with the nails pointing upward was left on the stairs from the basement to the first floor exposing employees to possibly stepping on the nails.

The worker was not protected from falling by the use of a guard rail system, a safety net or a personal fall arrest system.
