29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection.    (a)  Front of main building:  On or about 12/18/15, a Coleman Powermate 6875 portable generator was used for temporary 120v AC single-phase power to cord and plug connected equipment, including but not limited to:  halogen work light(s), string lights, water heater, and a skil saw; receptacles were not equipped with ground-fault circuit interrupters for shock protection.    (b)  Back of main building:  On or about 12/18/15, a Coleman Powermate 8000 and Honda EU6500is portable generator were used to provide 120v AC single-phase power to cord and plug connected equipment, including but not limited to: ventilation fans, string lights, halogen work lights; 15 and 20 Amp receptacles were not equipped with ground-fault circuit interrupters for shock protection.

29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact.  (a) Front of small building, east side:  On or about 12/18/15, employees were not using protective helmets and safety glasses while framing an OSB enclosure for a decon area; windy conditions increased the probability of a struck-by hazard.

(b) Small building, east side, basement:  On or about 12/18/15, 500 watt halogen work light was not equipped with a wire bulb guard, creating a burn or fire hazard.
