29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, nor by any of the means listed in subparagraphs (A) through (D) of this paragraph: a) Employees were exposed to electrical shock hazards because the employer did not ensure that the hanging electrical boxes in the asbestos abatement area had covers. Live, uninsulated wires were hanging out of the front of the boxes.

29 CFR 1926.405(g)(2)(iv): Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws: a) Employees were exposed to electrical shock hazards because the employer did not provide adequate strain relief for the cord used to power a vacuum during containment set up. The inner insulation was visible where the cord entered the plug.
