29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used. a) Bellestri's Home Improvement LLC on or about April 09, 2014, at the address of 10337 Roachton Rd. Perrysburg, OH: The employer did not ensure a means of access was provided to employees working from a ladder jack scaffold 18 ft. above the level below. The exposed employees were climbing the supporting ladders to access the platform. More than two feet separated the access ladder rung and the working platform.

Perrysburg, OH: The employer failed to ensure employees were protected from a fall while working from a ladder jack scaffold. Employees were exposed to a near 18' fall to the level below during roofing operations of a residential structure. No form of fall protection was in place.
