A) Bellestri's Home Improvements, LLC on or about December 27, 2018 at the address of 1106 Mccord Road, Holland, Ohio: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment.  Such programs would support the employer and employees in identifying and avoiding potential hazards, such as falls during job site operations, decreasing the probability of employee injury and/or illness.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.    A) Bellestri's Home Improvements, LLC on or about December 27, 2018 at the address of 1106 Mccord Road, Holland, OH: The employer had not trained employees in the appropriate use of fall protection, such as but not limited to, the use of personal fall arrest systems while performing various roofing operations. The employees did not recognize the hazards associated with working from an elevated locations of (9 and 21 feet) without the benefit of fall protection.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.     A) Bellestri's Home Improvements, LLC on or about December 27, 2018 at the address of 1106 Mccord Road, Holland, Ohio: The employer did not ensure employees were protected from falls while installing new metal roofing materials to the commercial structure. The employees were exposed to a potential 9 foot and 21 foot falls to the level below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet.    a) Bellestri's Home Improvements, LLC on or about December 27, 2018 at the address of 1106 Mccord Road, Holland, Ohio: The employer did not ensure the side rails of three 24 feet fiber glass extension ladder were extended at least three feet above the landing surface. Four employees were exposed to falls while using the ladder to access the scaffold and roof.
