29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) Juve Guzman - Findlay, Ohio: On or about April 9, 2014, at the address of 1500 Country Club Drive, Findlay, Ohio, the employer did not ensure the each employee engaged in roofing activities with unprotected sides and edges 6 feet more above the lower level was protected from falling. Employees working on a steep sloped (6:12) portion of a commercial structure roof did not have any form of fall protection in place, exposing employees to falls of approximately 13 feet to the concrete, which could result in permanent disability or death.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) Juve Guzman - Findlay, Ohio: On or about April 9, 2014, at address 1500 Country Club Drive, Findlay, Ohio, the employer did not ensure that employees had been trained in the appropriate use of fall protection, such as but not limited to the use of personal fall arrest systems. Employees were exposed to falls of approximately 13 feet to the concrete ground below, which could result in permanent disability or death.
