The violation was most recently observed at 450 Wellington Findlay, OH 45840 where employees were unable to identify fall hazards on the job site, exposing them to fall hazards of approximately 10ft. To abate this hazard the employer must ensure that its employees performing roofing work are trained to identify fall hazards prior to performing roofing work more than 6 feet high.

Juve Guzman, on or about May 16, 2018 at the address of 450 Wellington Findlay, OH: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness. Juve Guzman was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.503(a)(1) which was contained in OSHA inspection number1104065, citation number 1, item number 1a and was affirmed as a final order on March 23, 2016, with respect to a workplace located at 1005 Sunset Dr. Findlay, OH.
