Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by the load should the bridge/crane rails fail: a) Bldg. 1, on or about 4/15/14: The bridge rails rated for 6000 lbs had a MHC 1 die mover hoisting lift #B4 that was rated for 4000 lbs. b) Building 3, Maintenance Crib, on or about 4/15/14: The crane rail with a one ton Coffing overhead hoist attached had no rated load capacity markings on it. Abatement certification must be submitted for these items.

Abatement certification must be submitted for item b, c.
