On or about April 27, 2023, employees were operating the Webb roll-benders to bend metal. The employer did not ensure the point of operation was guarded exposing workers to amputation hazards. An employee suffered a multi-finger amputation.   b) Jaquith Industries Inc., 600 East Brighton Ave., Syracuse, NY - Building 3, Lathe Mills Area. On or about May 2, 2023, an employee was operating a Ridgid Drill to drill metal without the point of operation fully guarded. The guard was four inches above the point of operation. The employer did not ensure the point of operation was guarded exposing the worker to laceration hazards.  c) Jaquith Industries Inc., 600 East Brighton Ave., Syracuse, NY - Building 1,  Closing Area. On or about May 2, 2023, employees were operating the Reynolds Drills to drill cans. The employer did not ensure the point of operation was fully guarded. The guard was kept six inches above the point of operation. Workers were exposed to laceration hazards.
