29 CFR 1910.107(c)(6): Electrical wiring and equipment not subject to deposits of combustible residues but located in a spraying area as herein defined were not explosion-proof type approved for Class I, group D locations and did not otherwise conform to the provisions of subpart S of this part, for Class I, Division 1, Hazardous Locations:  STORAGE/WELDING SHOP; On or about April 9, 2014 the employer did not ensure that electrical equipment and wiring were of explosion-proof type approved for Class I, group D locations. Employees engaged in spray painting operations were exposed to fire and explosion hazards.

WELDING SHOP; On or about April 9, 2014 the employer did not ensure that the gap between a grinder's rest and abrasive wheel was 1/8 inch or less. This condition exposed employees to struck-by hazards.

The damaged area on the cable was wrapped with black electrical tap approximately two feet from the stinger. This condition exposed employees to hazards associated with electrical shock, arc, and sparks.

29 CFR 1910.305(c)(3)(ii): Single-throw knife switches, molded-case switches, switches with butt contacts, and circuit breakers used as switches shall be connected so that the terminals supplying the load are deenergized when the switch is in the open position. However, blades and terminals supplying the load of a switch may be energized when the switch is in the open position where the switch is connected to circuits or equipment inherently capable of providing a backfeed source of power. For such installations, a permanent sign was not installed on the switch enclosure or immediately adjacent to open switches that read, "WARNING -- LOAD SIDE TERMINALS MAY BE ENERGIZED BY BACKFEED."  WELDING SHOP; On or about April 9, 2014 the employer did not ensure that an energized knife safety switch panel was labeled with a warning sign as required by the standard and/or ensuring that the energized load side terminals were equipped with flash guards. This condition exposed employees to hazards associated with electrical shock, arc, and sparks.
