OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:  Well site:  On or about April 9, 2014, a service rig employee worked from a derrick mounted platform during the removal of a down well electric pump from an oil/gas well.  The provided emergency only escape system (the Geronimo) was not secured to a ground anchorage rendering it unusable.  This exposed the employee to the hazards of fire and explosion.
