29 CFR 1910.179(l)(3)(iii)(d): Pendant control stations was not kept clean and function labels kept legible:   a.  405 N. Clarendon Avenue, Scottdale, GA (Large Bay Area) - Employees were exposed to struck-by hazard.  The directional function label was missing from the control pendant with directs the moving of the heavy steel load.  There was no direction label as to the function and direction of the overhead gantry crane, on or  about 04/09/14.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazard from the damaged overhead crane hooks.   The hauling hooks on the overhead cranes were missing and/or had damaged safety latches designed to prevents equipment/steel beam from coming off the hook while in the midst of hauling the steel beam to areas of the shop, on  or about 04/09/14.   a.  Three-Ton Overhead Crane (1st Bay) - Safety latch spring pin is broken making the safety latch ineffective, loose and not properly secured.  There was a large gap/space where material can slide off  the hook.    b.  Ten-Ton Overhead Crane (Big Bay) - Safety latch spring pin is broken making the safety latch ineffective, loose and not properly secured.  There was a large gap/space where material can slide off the hook.   Among other methods, one feasible and acceptable method to correct this hazard is to follow the Hooks ASME B30.10-1993, Section 10-1.2.3 Maintenance.   Steel, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard Section 5(a)(1), which was contained in OSHA inspection number 947773, Citation number 1, Item number 1 and was affirmed as a final order on February 18, 2014, with respect to a workplace located at 405 N. Clarendon Avenue, Scottdale, GA 30079.

29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control:      a.  405 N. Clarendon Avenue, Scottdale, GA (Shop Area) - Employees were exposed to sudden energization or start-up of multi-energy source.  The company's lockout/tagout machine specific procedures did not include specific steps for testing the machines or equipment to determine and verify the effectiveness of lockout devices, tagout devices and other energy control measures.  Equipment includes but is not limited to, the Challenge Burn Table, Peddinghaus Anglemaster and Overhead Cranes, on or  about 04/06/14.      Steel, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(ii)(D), which was contained in OSHA inspection number 947773, citation number 1, item number 2b and was affirmed as a final order on February 18, 2014, with respect to a workplace located at 405 N. Clarendon Avenue, Scottdale, GA 30079.

29 CFR 1910.1200(f)(5)(i): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the identity of the hazardous chemical(s) contained therein:   a.  405 N. Clarendon Avenue, Scottdale, GA (Maintenance Shop) - Employees were exposed to chemical hazard.  Employees are storing hazardous chemical such as, flash solvent parts cleaner and lubricant oil into unmarked/unidentified, as to its content, plastic container bottles, on or about 04/09/14.
