29 CFR 1926.501(b)(10): "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone [i.e. without the warning line system] is permitted.  a. On or about April 11, 2014 - at the above addressed jobsite, an employee conducting flat roofing operations more than twenty (20) feet above the ground was not protected from falling.

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows:  a.  On or about April 11, 2014 - at the above addressed jobsite, an employee performing roofing operations more than twenty (20) feet above the ground had the personal fall arrest system secured to a storm water pipe on the roof, the anchor point used would not support at least 5,000 pounds, thereby exposing the employee to fall hazards.  b.  On or about April 11, 2014 - at the above addressed jobsite, two employees performing roofing operations more than twenty (20) feet above the ground had personal fall arrest systems secured to an angle bracket of an HVAC rooftop unit, the anchor point used would not support at least 5,000 pounds, thereby exposing the employees to fall hazards.
