29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems: On November 26, 2014 employees were not protected from a fall while roofing a commercial building with a steep sloped roof, exposing employees to a fall of 9.9 feet. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: undefined : On November 26, 2014 an employee used an extension ladder to gain access to a roof where the ladder did not extend three feet of extension past the roof edge, exposing the employees to a fall of 9.9 feet. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
