The most recent occurrence was at 7197 Heartland Drive, Dardenne Prairie, MO  63368, where they failed to make frequent or regular inspections of the worksite to ensure all workers were in compliance with company safety and health policies and OSHA standards.  The workers were exposed to the following hazards:    a) There was a fall hazard of approximately 9 feet, 6 inches as the crew worked on the roof without the use of an adequate fall protection system and/or fall arrest system at the front of the house.    b) There was a fall hazard of approximately 16 feet, 4 inches as the crew worked on the roof without the use of an adequate fall protection system and/or fall arrest system at the back of the house.    c) Employees failed to wear eye protection while operating nail guns to perform roofing operations.
