29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  a) The employer failed to provide documentation and certification for abatement of citation 1 item 1 in which a competent person did not conduct frequent and regular inspections of the jobsite. Condition noted on or about 4/10/14.  b) The employer failed to provide documentation and certification for abatement of citation 1 item 2 in which employees exposed to fall hazards in excess of 10 feet were not provided fall protection. Condition observed on or about 4/10/14.  c) The employer failed to provide documentation and certification for abatement of citation 1 item 3 in which employees had not received fall protection training. Condition noted on or about 4/10/14.  d) The employer did not provide documentation and certification for abatement of citation 1 item 4 in which employees were not provided a ladder to access the structure's upper floor. Condition observed on or about 4/10/14.  e) The employer failed to provide documentation and certification for abatement of citation 1 item 5 in which employees used ladder(s) for purposes other than for which they were designed. Condition observed on or about 4/10/14.  f) The employer failed to provide documentation and certification for abatement of citation 1 item 6 in which employees had not received any ladder safety training. Condition observed on or about 4/10/14.

The manufacturer of the A-framed ladder clearly states that the top two steps are not to be used.  As such, the exposed employees were exposed to fall hazards in excess of six feet to the ground surface below. Condition observed on or about 04/10/14.
