On or about April, 8, 2014 and times prior thereto, at the workplace located at or near 7725 County Road 142 in Blytheville, Arkansas, the coils being unloaded by the 888 crane from the CMT 123B barge were not secured in a manner that would prevent the coils from shifting and rolling, exposing the employees to struck by and drowning hazards.
