29 CFR 1918.83(a): When necessary to protect personnel working in a hold, the employer did not secure or block stowed cargo that was likely to shift or roll. On or about April, 8, 2014 and times prior thereto, at the workplace located at or near 7725 County Road 142 in Blytheville, Arkansas, the coils being unloaded by the 888 crane from the CMT 123B barge were not secured in a manner that would prevent the coils from shifting and rolling, exposing the employees to struck by and drowning hazards.

29 CFR 1918.100(e)(3): The employer did not review with each employee upon initial assignment those parts of the plan that the employee must know to protect the employee in the event of an emergency. On or about 8 April 2014 on or near the Kinder Morgan Bulk Terminal while unloading Barge CMT 123B the employer did not review with an employee parts of the emergency action plan relevant to his job assignment in a manner he was capable of understanding prior to unloading coils from barge hold. The conditions exposed employees to a delay in emergency egress.
