29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:       (a)  Sunrise Cooperative, Inc., Fremont, Ohio:  On or about November 27, 2013, at work site located at 1981 Fitchville River Road, Wakeman, Ohio, the employer did not ensure that while standing on top of the rail cars while breaking the door seals and opening the doors, employees were protected from falling.  The employees walked across the top of the cars, opened the doors on top of the cars, and stepped across to adjacent rail cars without the benefit of fall protection.  The employees were exposed to a fall of approximately 15 feet to compacted stone.
