29 CFR 1926.106(a): Employee(s) working over or near water where the danger of drowning existed, were not provided with U.S. Coast Guard approved life jacket(s) or buoyant work vest(s): a) During the period between April 12, 2014, and April 14, 2014, employees were exposed to the hazard of drowning while working over water at the junction chamber without having been provided with U.S. Coast Guard approved life jackets of buoyant work vests. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) During the period between April 11, 2014, and April 14, 2014, employees were exposed to the hazard of falling approximately 20 feet from unprotected sides/edges of a junction chamber. b) During the period between April 11, 2014, and April 14, 2014, employees were exposed to the hazard of falling approximately 12 feet while walking/working from the top of a concrete junction chamber wall. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
