29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: Northeast, cutting area, DeWalt DW716 miter saw: on or about April 16, 2014, employees cutting aluminum brackets and other pieces were exposed to amputation hazards where the unused portion of the rotating blade was not guarded during operation. The guard was tied to the operating handle disabling its purpose as designed. There is no abatement certification or documentation required for this item.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: Northeast, cutting and grinding areas, Coleman flexible cable and unmarked electrical extension cords: on April 16, 2014, employees were exposed to fire and burn hazards from possible equipment overheating and other malfunctions where listed and labeled electrical equipment was not installed and used in accordance with the manufacturers' provisions and other applicable listing standards for that type of equipment. Energized electrical flexible cable was connecting to other extension cords where prohibited. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
