29 CFR 1910.23(e)(1): Standard railing(s) did not consist of top rail, intermediate rail and/or posts and/or did not have a vertical height of 42 inches (106.7 cm) nominal from upper surface of top rail to floor, platform runway, or ramp level:    On or about 04/10/14, in the 2nd floor outside platform/loading area, two sections of platform's railings had vertical heights of 34 inches and 35 inches from upper surfaces of top rails to platform runway. Employees walking on this platform to load trucks from silos were exposed to falls below of approximately 14 feet.    ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent):    a) On or about 04/10/14, in the 2nd floor outside platform/loading area, there were not standard railings on the sides facing flour silos #5, #6, #8, #3 and #4 exposing employees to falls below of approximately 14 feet.     b) On or about 04/10/14, in the 2nd floor outside platform, there was not a standard railing on the platform leading to the emergency ladder between silos #1 and #5 exposing employees to falls below of approximately 14 feet.     ABATEMENT DOCUMENTATION REQUIRED    The Star of the West Mill Co was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.23 (c)(1), which was contained in OSHA inspection number 314456906, citation number 1, item number 1 and was affirmed as a final order on 1/24/2011, with respect to a workplace located at 162 N. Water St., Kent, OH, 44240.

The cover shall be securely held in place to prevent tools or materials from falling through:  a) On or about 04/10/14, on the four floor of the mill building, there were two holes of 3 inches and 4 1/4 inches wide on the floor/metal platform.
