29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) On or about 05/05/20, at the Star of the West Milling Company, located at 35 South Main Street, in Churchville, New York, the employer did not develop and document a Lockout/Tagout procedure for the Buhler Dust Collector which authorized employees can utilize when performing maintenance tasks such as, but not limited to, repairs, cleaning and unjamming flour during the milling process. The Buhler Dust Collector has both pneumatic (Air Lock) and electrical (Mill Filter Screw) energy sources.  Employees are exposed to amputation injuries.  b) On or about 05/05/20, at the Star of the West Milling Company, located at 35 South Main Street, in Churchville, New York, employees engaged in unjamming or unclogging "bridged" flour in a Buhler Dust Collector did not shut down, de-energize and lock out this machine.  Employees were exposed to amputations hazards due to moving parts (rotary valve) inside the Buhler Dust Collector.   ABATEMENT DOCUMENTATION REQUIRED
