29 CFR 1926.754(e)(5)(i): Except as provided in Sec. 1926.760(c), metal decking was not laid tightly and immediately secured upon placement to prevent accidental movement or displacement. a) Harvestland Constructors, Inc. on or about April 11, 2014 at the address of 5968 State Hwy 199 Carey, OH: The employer failed to ensure metal decking (steel grating) being installed on the second level in the mill tower was immediately secured upon placement. Employees working from unsecured deck panels were exposed to fall hazards of approximately 17-6 in that fasteners were not installed preventing unintentional displacement.

29 CFR 1926.760(d)(1): Guardrail systems, safety net systems, personal fall arrest systems, positioning device systems and their components did not conform to the criteria in Sec. 1926.502 (see Appendix G to this subpart). (a) Harvestland Constructors, Inc. on or about April 11, 2014 at the address of 5968 State Hwy 199 Carey, OH: The employer failed to ensure that the employer fabricated horizontal lifeline was designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system. Employees utilizing the horizontal lifeline during steel decking operations were exposed to potential falls in that the employers design and installation of the system did not incorporate engineering data, identifying the specifications of installation and limitations of use. Employees were unable to effectively assess the parameters in which the protective system may be safe to utilize.

29 CFR 1926.1424(a)(2)(ii): 29 CFR 1926.1424(a)(2)(ii): To prevent employees from entering hazard areas, the employer did not erect and maintain control lines, warning lines, railings or similar barriers to mark the boundaries of the hazard areas and/or did not train each employee to understand what these markings signify. a) Harvestland Constructors, Inc. on or about April 16, 2014 at the address of 5968 State Hwy 199 Carey, OH: The employer failed to ensure the counterweight swing radius of the Manitowoc model 10000 lattice boom crane was adequately barricaded, marking the boundaries of the hazard area. Employees working in and around the cranes superstructure were exposed to stuck-by and crushed-by hazards.
