29 CFR 1904.29(b)(1): 29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in the detail as required by the regulation: a) On April 16, 2014, and times prior thereto, at the California Cartage warehouse in Port Wentworth, GA, the employer failed to provide adequate details in Column E of the 2014 OSHA 300 form, demonstrated by including generic locations such as Warehouse and Trailer. b) On April 16, 2014, and times prior thereto, at the California Cartage warehouse in Port Wentworth, GA, the employer failed to provide adequate details in Column F of the 2014 OSHA 300 form, demonstrated by including generic injuries such as Contusion and Strain. ABATEMENT CERTIFICATION IS REQUIRED

29 CFR 1910.1200(h)(3)(iv): The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information: a) On April 16, 2014, and times prior thereto, at the California Cartage warehouse in Port Wentworth, GA, the employer failed to provide effective information and training pursuant to the Hazcom 2012 requirements effective December 1, 2013, exposing employees to various hazards related to the hazardous chemicals to which each was exposed. ABATEMENT CERTIFICATION IS REQUIRED
