29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface were not used:    On or about April 14, 2014, the employer failed to ensure employees working from a ladder jack scaffold placing weatherproofing on the side of a building had a safe access onto the scaffold.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    On or about April 14, 2014, the employer failed to ensure employees working from a ladder jack scaffold placing weatherproofing on the side of a building were protected from a fall of approximately 15 feet 4 inches by a fall protective system/device.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  On or about April 17, 2014, the employer failed to ensure employees working from a 3/12 pitch roof were protected from a fall of approximately 18 feet by using an adequate fall protective system/device. Employees were performing tear off and replacement of weatherproofing on a commercial roof.
