29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about October 14, 2020, during the installation of roofing material at a job site located at 3838 Boettler St NE Canton, OH 44721; the employer did not ensure employees working on a roof of a single family home were protected from a fall of approximately 10.2 feet by utilizing a fall protection system. McCurry Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b) (13) Duty to have fall protection), which was contained in OSHA inspection number 1311709, citation number 01, item number 01 and was affirmed as a final order on 05/29/2018, with respect to a workplace located at 3917 Marshview, Portage Lakes, Ohio 44319 McCurry Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b) (13) Duty to have fall protection), which was contained in OSHA inspection number 1475977, citation number 02, item number 01 and was affirmed as a final order on 6/23/2020, with respect to a workplace located at 6424 Shadygrove Cir NE, Canton Ohio 44721
