Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    (a)  501 SOUTH 29TH STREET, HARRISBURG, PA, ROOFTOP - Employees performed roofing work on a 4/12 pitch roof where the employees were wearing fall protection harnesses, but were not attached to the horizontal lifeline that had been installed, thereby exposing employees to fall hazards that ranged from approximately twenty-two (22) to thirty-two (32) feet to the ground below, on or about April 17, 2014.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
