29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    (a)  501 SOUTH 29TH STREET, HARRISBURG, PA, ROOFTOP - Employees performed roofing work on a 4/12 pitch roof where the employees were wearing fall protection harnesses, but were not attached to the horizontal lifeline that had been installed, thereby exposing employees to fall hazards that ranged from approximately twenty-two (22) to thirty-two (32) feet to the ground below, on or about April 17, 2014.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the correct use and operation of personal fall arrest systems, guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used:    (a)  501 SOUTH 29TH STREET, ROOFTOP - Employees performed roofing work at heights ranging from approximately twenty-two (22) to thirty-two (32) feet, and the employees had not received adequate training on the use of the fall protection systems they were using, thereby exposing employees to serious injuries, on or about April 17, 2014.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
