On 16 April 2014, a worker was performing roofing activities on the roof of a single story residence under construction without the use of fall protection, exposing the worker to a fall hazard of approximately 9 feet to the ground.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access. On 16 April 2014, two workers were performing roofing activities on a residence under construction and accessed the roof with a portable ladder that was not extended at least 3 feet above the upper landing, exposing the workers to a fall hazard of approximately 9 feet.
